About Your Recovery
A Different Kind of ACL Rehab
An ACL injury is one of the most disorienting experiences an athlete can face. Whether yours happened on the field, in the gym, or during an everyday activity, the questions are often the same: How long will this take? Will I get back to full strength? And who is actually qualified to guide me through this? If you're searching for ACL rehab in Buffalo, you've landed in the perfect place to begin your recovery journey.
At Impulse Physical Therapy, you work directly with Dr. Alex Peters or Dr. Kathryn Piraino, one-on-one, every session, using objective testing technology and evidence-based protocols from your first post-op visit through full return to sport.
Dr. Peters is WNY's only Board-Certified Sports Clinical Specialist, a credential held by less than 0.5% of physical therapists nationally, and the clinical foundation of every ACL program at Impulse PT.
The Truth
Why Most ACL Rehab Falls Short
ACL rehabilitation is a specialty, one that requires thousands of hours of focused clinical training and familiarity with current return-to-sport research. Most ACL rehab in Buffalo follows the same generic template regardless of the patient. Standard PT education does not adequately cover it, and most high-volume clinics are not built to deliver it well.
Generic protocols. Most clinics use the same ACL program for every patient regardless of sport, age, or surgical technique, leading to higher re-injury rates.
No objective testing. "How does it feel?" is not a return-to-sport criterion. Without force plate testing and strength measurements, clearance is based on time, not data.
Shared sessions. When your therapist is managing three other patients simultaneously, the quality and focus of your care is fundamentally compromised.
Outdated protocols. Some ACL programs still in use today are based on research from the 1980s and 90s. The science has moved. Your rehab should reflect that.
Return-to-sport decisions should be based on data, not a calendar.
Our 3-Phase Approach
How We Guide Your ACL Recovery
Every ACL patient at Impulse PT goes through a structured three-phase process, but the program within that structure is built entirely around you. Your sport, your surgical history, your movement patterns, your goals.
01
Evaluation and Goal Setting
A comprehensive biomechanical assessment beyond the standard screen. We review your surgical report, assess movement under load, establish objective baselines, and build the plan around where you want to get to, not just where you are.
02
Strength and Neuromuscular Training
Individualized programming that progresses based on objective data, not a calendar. Targeted quad, hamstring, and hip strengthening. Force plate testing. 3D movement analysis. Dynamometry at every phase. Sport-specific drills introduced as benchmarks are met.
03
Return-to-Sport Clearance
Clearance decisions at Impulse PT are based on objective criteria: limb symmetry indices, hop testing, strength ratios, and sport-specific movement assessments. You are cleared when the data says you are ready.
Unique to Impulse PT
The ACL Bridge Program
If your surgery is scheduled for a future date, our ACL Bridge Program is one of the most valuable investments you can make in your ACL rehab. Available to Buffalo NY patients before and after surgery, this pre-surgical protocol sets you up for a significantly faster recovery.
Research consistently shows that patients who are stronger going into ACL surgery recover significantly faster and with better outcomes coming out. The Bridge Program is a pre-surgical strengthening protocol designed to optimize quadriceps strength, single-leg stability, and neuromuscular control before your procedure.
Prehabilitation before ACL surgery has been shown to reduce recovery time and improve return-to-sport rates. Ask us about whether the Bridge Program is right for your timeline.
ACL recovery involves a lot of conversations, and not all of them are honest ones. Here is what you will never hear from us, and why it matters.
"Just rest and it will heal."
Rest alone does not rebuild the strength, neuromuscular control, or movement quality ACL rehab requires. Passive recovery without progressive loading is one of the primary drivers of re-injury.
"Avoid everything that hurts."
Pain avoidance leads to compensation patterns that create new problems elsewhere. We teach you how to interpret pain, modify load appropriately, and keep training through recovery wherever possible.
"You're cleared, you're good to go."
Clearance without objective data is not clearance. We do not sign off on return to sport based on how your knee feels. We base it on what the force plates, dynamometry, and functional testing show.
"This is as good as it gets."
With the right program and enough time, the majority of athletes return to full sport at or above their pre-injury level. We have guided professional athletes, competitive youth players, and active adults through full recoveries.
"Come back three times a week indefinitely."
Our goal is to make you independent. We give you the knowledge, the program, and the data to understand your own recovery, so you are not dependent on us longer than necessary.
"Give up what you love."
Quitting your sport or activity is rarely the only path to recovery. We find ways to keep you connected to what drives you, modifying training where needed rather than eliminating it.

ACL rehabilitation and return-to-sport testing are among his primary clinical focuses.
